    We send you recalls/reminders by SMS/mail for follow up consultations and test results

    Independent practitioners at the Centre are committed to preventative health and support Commonwealth and State Reminder programs. This means we may occasionally send you information by SMS/mail on these programs such as Vaccinations and Cervical Screening

    If you do not want to receive these reminders, advise your Doctor, Allied Health Practitioner or reception

Transferring records from a ForHealth practice

This applies if you are:

  • Requesting records held in relation to you ( Ages 14 years and over)
  • Requesting medical records for a child aged 13 years and under (parent or legal guardian)
  • Requesting records for a deceased patient (authorised Executor)
  • Guardian/Attorney requesting records on behalf of a patient
  • Requesting records to be sent to a Medical Centre not owned by Forhealth

What we need to process your request

  • A completed online request form with Signed consent and proof of identity (which will require your signature and consent, as well as proof of identification)
  • payment of an administrative fee (our team will email or post you an invoice)
  • please allow up to 5 weeks for processing

Do you Bulk-Bill?

All GPs at the Centre are independent practitioners who operate their own practice and who decide their own fees for their own patients. Generally speaking, the GPs at this Centre are mixed billing, which means only some services and patients are Bulk Billed. Please speak with your Doctor to determine their fees.

What is mixed billing?

Mixed Billing means independent GPs charge a private fee for some consultations and patients, and bulk bills others. Each GP makes his or her own decisions on billings. Bulk-Billing is when the payment for your consultation or treatment with your doctor is covered entirely by Medicare, whereas services not covered by bulk billing attract a private fee and you will receive a rebate from Medicare. This means there is an out of pocket cost to you.

Why should I pay a private fee?

The GPs at the Centre passionately believe in providing access to high quality healthcare to the community. By becoming a mixed billing practice, independent GPs are able to meet the rising costs of running their own practice to minimise wait times and provide high quality care so that you and your family receive the healthcare that you deserve in a family friendly environment.

No Medicare Card

Patients without a valid Medicare or concession card will pay a private fee and are not eligible to receive a Medicare rebate.
